Required New York State disclosures

Two forms every New York buyer must receive in writing before working with a broker.

This is not a contract. New York State law requires brokers to explain in writing how they will represent you, and to provide notice of your fair housing protections. Review the two summaries below, open the full forms, and sign once at the bottom to acknowledge receipt.

1. New York State Disclosure Form for Buyer and SellerDOS-1736-f (Rev. 09/21)

This form is provided to you by your LEVEN advisor of LEVEN Real Estate, a licensed real estate broker acting in the interest of the buyer, as buyer’s agent. Your signature is an acknowledgment of receipt only.

A buyer’s agent represents you and your interests throughout the purchase, including negotiating the price and terms on your behalf. When acting as your agent, LEVEN owes you loyalty, a duty of care, confidentiality, obedience, and full disclosure. A buyer’s agent does not represent the seller’s interests. 2. New York State Housing and Anti-Discrimination Disclosure FormDOS-2156 (11-25)

Federal, state and local fair housing laws make it unlawful to discriminate in housing on the basis of race, creed, color, national origin, sexual orientation, gender identity or expression, military status, sex, age, disability, marital status, lawful source of income or familial status.

A broker or salesperson breaks the law by misrepresenting that a property is unavailable, negotiating different terms, steering you toward or away from particular neighbourhoods or buildings, blockbusting, or expressing any discriminatory preference in any advertisement, inquiry or record.

If you believe you have been the victim of housing discrimination you may file a complaint, free of charge, with the NYS Division of Human Rights (dhr.ny.gov, Fair Housing hotline (844) 697-3471) or with the NYS Department of State, Division of Licensing Services ((518) 474-4429). It is unlawful for anyone to retaliate against you for filing a complaint.
